Useful inputs also connect the event: what happened, the observable behavior, the intervention, and the response. You do not need to write a full paragraph in every field. Short, accurate facts give the drafting tool enough structure while keeping the workflow fast.
What the tool should never decide for you
An AI drafting tool should not invent clinical events, determine medical necessity, select interventions, verify credentials, or approve documentation.
